Hi Cadence! What were your expectations coming into the Medical internship?
Coming into this international medical internship, I didn’t have any specific expectations, as I had no idea what an experience in healthcare would look like, especially without any prior certifications or significant clinical experience. However, I was pleasantly surprised by the variety of tasks I was allowed to participate in.
As an intern in Costa Rica, I had the opportunity to shadow doctors and assist with responsibilities such as cleaning wounds, positioning lights and obtaining supplies for the doctor during procedures, positioning patients, wrapping dressings, and more. I also gained hands-on experience using a stethoscope and with physical examination techniques. Additionally, I observed numerous consultations, exams, and procedures that I had not anticipated being able to witness. There were even a couple times I got to act as a translator for English-speaking patients!
How did you adapt to the local culture during your internship?
During the internship, I learned to adapt to local customs and expressions that aren’t typically taught in formal language classes - for example, using “permiso” for “excuse me” and “con gusto” instead of “you’re welcome.” Although local dress wasn’t drastically different, I made a conscious effort to dress modestly and appropriately for each environment.
I also adapted to cultural attitudes toward nature - for instance, avoiding the killing of insects indoors, which differs from what I was used to. Respecting these values helped me grow more open-minded and adaptable. These behavioral adaptations demonstrated my ability to adjust professionally across cultures and contexts.
How did the internship help you progress toward your long-term goals?
Throughout the internship, I significantly improved my Spanish, especially in a professional context and in understanding medical terminology. My clinical skills also advanced - particularly in conducting exams, using a stethoscope, and observing how physical exams are performed. I was exposed to a wide variety of diagnostic exams, including physical examinations, ultrasounds, x-rays, blood tests, and EKGs, as well as procedures like IV insertions, injections, stitches, and PRP treatments.
My knowledge of different conditions, diagnostic methods, and how to communicate with patients expanded considerably. By closely observing a physician’s daily workflow, I learned what it truly means to be a doctor - not only in terms of medical knowledge, but also in how to interact with and care for patients. As someone pursuing a career in medicine, this experience was both deeply relevant and incredibly motivating. It not only strengthened my medical school application but reinforced my commitment to a future in the healthcare field.
How did you ensure effective communication throughout the internship?
To ensure effective communication, I consistently asked questions to clarify meaning and confirm my understanding. When speaking in my native language, I made an effort to slow down and simplify vocabulary to help non-native speakers follow along. I also utilized technology to bridge language gaps, especially when dealing with complex terms or concepts. Overall, I found that open communication, active listening, and constant clarification were essential to keeping everyone on the same page.
How did this internship empower your journey toward a medical career?
One of the most empowering aspects of this medical internship was getting a first-hand look at what it’s like to be a doctor. I learned about the full process - from recording a patient history and checking vital signs to conduct physical exams, running diagnostic tests, and ultimately arriving at a diagnosis.
Completing this internship in Costa Rica both prepared and inspired me to become a physician who truly listens to and connects with their patients. I was especially struck by how sometimes all a patient needs is someone to listen to them. Seeing the wide range of conditions and treatments only deepened my fascination with medicine and reinforced my passion for the field. I left the experience feeling more confident, motivated, and excited for the path ahead.
What advice would you give to future interns?
The most important advice I would give to someone preparing for a similar internship is to stay open-minded. Embrace every opportunity and don’t hesitate to try new things. Ask questions freely and don’t worry if you don’t understand something right away. Always remain respectful of the culture and avoid making comparisons that label one way of life as better than another; they are simply different. Although at times things may be uncertain at times, learn to not only exist in the uncertainty but thrive in the unknown. Understand that not everything will feel comfortable, but that you will not grow unless you feel uncomfortable. This mindset will enrich your experience and help you grow both personally and professionally.
